  • Student Declaration

    I meet the eligibility criteria for this program being:

    • I do not hold, nor am I currently undertaking a Certificate III level or higher qualification with another training organisation. (With the exception of Certificate II qualifications completed at high school)
    • I am 15 years of age or older and no longer at school
    • I am an Australian/New Zealand citizen or a Permanent Resident Visa Holder.
    • I am a refugee/humanitarian Visa Holder or a temporary resident on a demonstrated pathway to Permanent Residency.
    • I permanently reside in Queensland.

    I understand that:

    • after I complete this qualification, I will no longer be eligible for subsidized training at a Certificate III level through this program.
    • I am required to make a co-contribution to participate in this training and am aware of what this co-contribution payment is.
    • I will be required to pay this co-contribution fee is full or part prior to commencement of my training.
    • If applicable, I am required to have formally withdrawn from all other funded courses, and I have notified the other Training Organisation. If required I will provide a Statement of Attainment to show all results of these studies.
    • Upon completion or discontinuation of my studies, I will be required to complete a mandatory training and employment survey

    I do not hold, nor am I currently undertaking a qualification as part of an apprenticeship where I have accessed User Choice funding at the same level or higher.

    I understand that I am required to make a co-contribution to participate in this training.

    I have formally withdrawn from all previous ‘funded’ courses, and I have notified other RTO’s of this. If required, I can provide a Statement of Attainment to show all results of my previous studies.

    I can and will provide a Statement of Attainment for any previously completed studies.

    I am an Australian/New Zealand citizen studying in Australia or an overseas resident studying in Australia.

    I confirm that I have been provided with all the information I need to make an informed decision about my enrolment into this qualification or short course.

    I understand that there are fees payable to undertake this training and that these fees are due in full or part prior to commencement.

    I have accessed, read and understood the terms and conditions of enrolment.

    I have accessed, read and understood the Student Handbook and also understand I can access this at all times through the RTO website.

    I acknowledge that I have been advised and understand the requirements of the qualification and funding I have enrolled in.

    I understand that I must provide all eligibility documentation before commencing training and that Core Industry Training reserve the right to terminate my enrolment and re-allocate my position should I not make reasonable progress through my course during the term of the contract.

    I understand that CSQ funding is available for a limited term.

    I consent for my details to be provided to CSQ for the purposes of (but not limited to) a review, conducting participant surveys or being contacted for the purposes of advise regarding CSQ products and services.

    Eligibility

    An Eligible Worker means:

    (a) an Australian or New Zealand citizen; or

    (b) a permanent resident of Australia; or

    (c) a refugee and humanitarian visa holder.

    AND MUST:

    (d) permanently reside in Queensland; or

    (e) be permanently employed in Queensland.

    Definitions

    An Eligible Worker has the same meaning as the Building and Construction Industry (Portable Long Services Leave) Act 1991 (Qld) as amended from time to time.

    An Unemployed Eligible Worker is a person who has been unemployed for a period of up to, but not greater than, four years that would otherwise meet the requirements of an Eligible Worker.

    You are not eligible for this funding is you meet any of the below criteria:

    (a) an Employee of any Authority (excluding employees of an Indigenous Council that is a Principal Contractor);

    (b) currently enrolled and participating in a Queensland secondary school program (excluding a school based Apprentice or Trainee);

    (c) a contracted trainer or assessor or existing worker of an RTO;

    (d) previously funded under this Program in the same Contract Term;

    (e) funded by an Authority or such other source for delivery of the same Training being undertaken as part of this Program;

    (f) those Participants seeking a Verification of Competency (or Determination of Competency) services under this Agreement

    Higher Level Skills – General Construction Only

    An Eligible Worker, includes relevant clerical, administration and professional staff whose employer is part of the building and construction industry or an Unemployed Eligible Worker.

    AND MUST NOT BE:

    eligible under the Apprentice Advance Plus.

    Apprentice Advance Plus Eligibility Requirements

    Participants must be:

    (a) a current active Apprentice or Trainee in Queensland undertaking a Recognised Apprenticeship or Traineeship, who is in the 3rd or 4th year of their Apprenticeship or Traineeship; or

    (b) a current active Apprentice or Trainee in Queensland undertaking a Recognised Apprenticeship or Traineeship, who is in the 3rd or 4th year of their Apprenticeship or Traineeship and working on a Major Project; or

    (c) an Eligible Worker, including an eligible Participant who is working on a Major Project or an Unemployed Eligible Worker in the first year out of their Recognised Apprenticeship or Traineeship who must commence training within 365 days of the completion date in DELTA; or

    (d) a cancelled Apprentice or Trainee who was in their 3rd or 4th year undertaking a Recognised Apprenticeship or Traineeship that has been cancelled and they are currently accessing State Government funding to complete their institution-based training.

    Skills Gap Training and Assessment Funding Only

    Student must be able to demonstrate RPL in at least 80% of the program.

    I confirm the accuracy of the information provided; I give my permission for Core Industry Training to act on my behalf to contact ASQA or any Registered Training Organisation I have been previously enrolled with to obtain information about my enrolment status and past academic results (i.e. withdrawal and/or Statement of Attainment). Any fees charged by the other RTO/s will be my expense. I have read and understood the Core Industry Training Handbook including the Refund and Complaints Policies and Processes. I acknowledge this is available at www.coreindustrytraining.edu.au at all time for my reference. I understand that my training may be delivered by a Core Industry Training approved on Core Industry Trainings’ behalf. I consent to the disclosure of my details by the RTO to government agencies as required. Please read the Privacy Notice and Student Declaration. I will abide by my own responsibilities as a student of Core Industry Training as outlined in the Student handbook. I have received and understood the terms and conditions of enrolment. I will fill out and return a Student Survey following the completion of my qualification. I have been informed of how my training and assessment will be conducted. I declare that all the information provided in this enrolment is true and correct and I am aware that consequences that may occur for providing false, misleading or incomplete information may be cancellation of my enrolment or withdrawal of any offer made by Core Industry Training.
